SMS Opt-In Proof Artifacts — Three Consent Methods
This page is published as the public proof artifact referenced from the Twilio campaign Message Flow / Call To Action field for the Wolf's Edge Integrators Number 1 Leader SMS Alignment Check-Ins program. Because this is a closed B2B program with no public self-subscribe web form, the three sections below show the verbatim disclosure language and verbatim sample capture for each of the three opt-in consent methods used during onboarding: (1) written authorization, (2) direct email from the recipient's own email address, and (3) documented verbal authorization on a recorded or logged onboarding call.

Program: Wolf's Edge Integrators Number 1 Leader SMS Alignment Check-Ins

Operator: Wolf's Edge Integrators

Support: sales@wolfsedgeintegrators.com

Public program details: https://www.n2ls.app/sms-compliance

Method 1 — Written Authorization (Paper Form)

Blank illustrative copy of the written opt-in authorization that each Number 1 Leader completes during onboarding. The Wolf's Edge Integrators representative presents this form to the recipient, the recipient signs, and the form is archived as the consent record.

1a. Recipient information and SMS opt-in checkbox

Full name:

Role at organization (must be Number 1 Leader, Owner, or Visionary):

Client organization:

Recipient's email (must be the recipient's own email address):

Mobile number for SMS check-ins:

I agree to receive recurring Number 1 Leader SMS Alignment Check-In text messages from Wolf's Edge Integrators at the mobile number provided above. Message frequency is about every other week. Message and data rates may apply. Reply STOP to opt out, HELP for help. Consent is not a condition of purchase. See our Privacy Policy and Terms of Service.

The recipient checks this box themselves to voluntarily opt in. It is shown unchecked in this blank example.

1b. Exact opt-in disclosure

By signing below, the recipient affirmatively agrees to the disclosure below. A Wolf's Edge Integrators representative reads this disclosure aloud or presents it in writing to the recipient before consent is captured.

“By providing my mobile number, I agree to receive recurring Number 1 Leader SMS Alignment Check-Ins from Wolf's Edge Integrators. Message frequency is about every other week. Message and data rates may apply. Reply STOP to opt out or HELP for help. Consent is not a condition of purchase. Mobile information will not be shared with third parties or affiliates for marketing or promotional purposes. Terms: https://www.n2ls.app/terms-of-service. Privacy Policy: https://www.n2ls.app/privacy-policy.”

1c. What the recipient will receive

Recurring operational SMS alignment check-ins similar to the example below. The messages are service messages, not marketing.

Wolf's Edge Integrators: Reply with one number from 1 to 10 for how aligned you feel about Jane Smith as Integrator for Acme Co right now. Reply STOP to opt out or HELP for help.

Replying STOP opts the recipient out. Replying HELP returns support contact information.

1d. Recipient signature and consent capture

Recipient signature:

Printed name:

Date of consent:

The Wolf's Edge Integrators representative records the recipient's name, mobile number, opt-in status, consent source (“written authorization”), and date of consent in the Number Two Leadership System. SMS sending is gated server-side and refuses to send unless the contact is marked opted in.

Method 2 — Direct Email From The Recipient's Own Email Address

When consent is captured by direct email, a Wolf's Edge Integrators representative sends the exact disclosure email below to the recipient's own email address. The recipient must reply affirmatively from that same email address. Both messages are archived as the consent record in place of a signed form.

2a. Sample disclosure email sent to recipient

From: sales@wolfsedgeintegrators.com
To: [recipient's own email address on file]
Subject: Confirm enrollment — Number 1 Leader SMS Alignment Check-Ins

Hi [Recipient first name],

enable SMS to your number, please reply to this email from this same email
address with "I agree" to confirm consent to the following disclosure:

"By providing my mobile number, I agree to receive recurring Number 1 Leader
SMS Alignment Check-Ins from Wolf's Edge Integrators. Message frequency is
about every other week. Message and data rates may apply. Reply STOP to opt
out or HELP for help. Consent is not a condition of purchase. Mobile

or promotional purposes. Terms: https://www.n2ls.app/terms-of-service.
Privacy Policy: https://www.n2ls.app/privacy-policy."

Mobile number on file: [recipient's mobile number]

If the mobile number above is incorrect, reply with the correction along
with your "I agree".

Thanks,
[Wolf's Edge Integrators representative name]
Wolf's Edge Integrators
sales@wolfsedgeintegrators.com

2b. Sample affirmative reply from recipient's own email address

From: [recipient's own email address]
To: sales@wolfsedgeintegrators.com
Subject: RE: Confirm enrollment — Number 1 Leader SMS Alignment Check-Ins

I agree.

The mobile number on file is correct.

[Recipient name]

Both emails are archived. The Wolf's Edge Integrators representative records the recipient's name, mobile number, opt-in status, consent source (“direct email from recipient's own address”), the recipient's reply timestamp, and date of consent in the Number Two Leadership System.

Method 3 — Documented Verbal Authorization On A Recorded Or Logged Onboarding Call

When consent is captured by direct verbal authorization, a Wolf's Edge Integrators representative reads the exact disclosure verbatim on a recorded or logged onboarding call and captures the recipient's affirmative verbal reply. The recording (or contemporaneous call log) is archived as the consent record in place of a signed form.

3a. Verbatim sample transcript of the verbal consent capture

Wolf's Edge Integrators representative
(on a recorded or logged onboarding call):

"Before I enroll your mobile number in the Number 1 Leader SMS Alignment
Check-Ins, I need to read you the opt-in disclosure and capture your verbal
agreement on this call. Here is the disclosure:

By providing my mobile number, I agree to receive recurring Number 1 Leader
SMS Alignment Check-Ins from Wolf's Edge Integrators. Message frequency is
about every other week. Message and data rates may apply. Reply STOP to opt
out or HELP for help. Consent is not a condition of purchase. Mobile
information will not be shared with third parties or affiliates for marketing
or promotional purposes. Terms are available at n2ls.app/terms-of-service
and the Privacy Policy is at n2ls.app/privacy-policy.

Do you affirmatively agree to receive these alignment check-ins on the
mobile number you provided?"

Recipient:

"Yes, I agree."

Wolf's Edge Integrators representative:

"Thank you. I'm recording your verbal consent now along with today's date
and the mobile number on file."

After the call, the Wolf's Edge Integrators representative records the recipient's name, mobile number, opt-in status, consent source (“documented verbal authorization”), date of consent, and the call recording or call-log reference ID in the Number Two Leadership System.
